But few words are necessary in explanation of tho method I have followed in editing this volume of selections from tho clinical works of Duchonne do Boulogne.Dr. Tibbits' well-known translation of tho first part of L'Electrisation Localisée has made it unnecessary for the New Sydenham Society to undertake the translation of any of Duchonno's writings which treat of purely electrical subjects. Tho ensuing chapters, therefore, will bo found to be almost exclusively devoted to those clinical observations by which the author's great professional reputation was mainly established.The reader cannot fail to notice that, with very few exceptions, the selections are taken from the last edition of L'Electrisation Localisée. The reason of this is obvious. Duchonne's observations were originally published separately, or in one or other of the French medical journals, but almost all of thorn became ultimately incorporated with tho book which to the last retained its inadequate and rather misleading title of L'Electrisation Localisee. Each edition of this work was bigger than the one which preceded it, its growth being mainly duo to tho addition of those papers on clinical, physiological, and pathological subjects which the author had written in the interval which elapsed between the editions.It will be observed that there are no selections from the Physiologic des Mouvements, which by many would bo considered the greater of Duchenne's two groat works.

Whenever it is possible, Dr. Barnardo is allowed to speak for himself. Naturally the chief place is given to his public work, but at every point appears his attractive personality.
There is a county in the west of England not so full of life, indeed, nor so widely spoken of as some of its manufacturing leviathan brethren in the north but which is, nevertheless, very dear to those who know it well. Its green pastures, its waving wheat, its deep and shady and — let us add — dirty lanes, its paths and stiles, its tawny-coloured, well-built rural churches, its avenues of beeches, and frequent Tudor mansions, its constant county hunt, its social graces, and the general air of clanship which pervades it, has made it to its own in habitants a favoured land of Goshen. It is purely agricul tural agricultural in its produce, agricultural in its poor, and agricultural in its pleasures. There are towns in it, of course depots from whence are brought seeds and groceries, ribbons and fire-shovels in which markets are held and county balls are carried on which return members to parliament, gener ally — in Spite of reform bills, past, present, and coming — ih accordance with the dictates of some neighbouring land magnate: from whence emanate the country postmen, and where is located the supply of post-horses necessary for county visitings. But these towns add nothing to the im portance of the county; they consist, with the exception of the assize-town, of dull, all but deathlike single streets. Each possesses two pumps, three hotels, ten shops, fifteen beer-houses, a beadle, and a market-place.
The capacity of professional medicine to resist change - and also concordance - is impressive, but perplexing. It is one of the issues I seek to address in this book. I suggest that a preoccupation with trying to change the relationship between the professional-patient dyad has deflected attention from the extent to which such relations are embedded in, and constrained by, wider administrative and organisational structures, especially as these relate to the operation of professional hierarchies and interprofessional deference and allegiances. Barriers to change also result from the inertia of a system which has evolved a highly stylised etiquette as an adaptive mechanism to contain the difficulties and tensions intrinsic to the medical consultation. Its therapeutic purpose and potential are often subordinate to the goal of achieving success as a social encounter. The principles of concordance are deeply challenging to traditional professional roles and status. However, medicine has always displayed an ability to block change through tactics of appropriation and incorporation. Professionals have often shown particular difficulty giving up their monopoly of 'expertise' and in acknowledging the legitimacy of the patient perspective. Although the term 'concordance' has become quite widely used, its meaning is usually subverted by its employment as a synonym for 'compliance', albeit 'informed' compliance. A slightly more sophisticated version values professional elicitation of the patient perspective in order to more accurately tailor information as a means of overcoming the unhelpful m/sconceptions that impede compliance. The original emphasis on the consultation as a negotiated exchange, in which the professional has something of value to learn from the patient, has largely been lost. The rhetoric of modernity and change provides an effective mask for inertia and conservatism. Preface.
