Following the conquering of the highest mountain tops in the world, biomedical studies under extreme physiological and specific altitude-dependent pathologic conditions could be carried out. This issue of 'Respiration' summarizes the most important medical experiences gathered in extreme altitude environment, including new aspects in physiology, new diseases, and atypical responses. These are presented by researchers who are not only experts in the field but even contributed to these efforts as members of expeditions in extreme altitude.
