A large variety of chronic renal diseases exhibit an apparently relentless progression to renal failure. In order to develop new prophylactic or therapeutic approaches, both cellular and humoral pathogenetic mechanisms have been studied intensely; this special issue attempts to provide a state-of-the-art overview of such mechanisms. First, the question of how glomerular, mesangial, endothelial and/or podocyte injury contributes to renal failure is dealt with, followed by a discussion of the emerging field of how macrophages can be programmed to exhibit different ‘phenotypic’ activities. Moreover, two articles address the question of how primary glomerular damage is propagated into the renal tubulointerstitium, succeeded by reviews of the pathogenetic roles of tubular and interstitial cells. Finally, given the rapid increase of diabetic nephropathy, a last review is devoted to insights into this disease derived from cell culture and in vivo studies. This publication is aimed at all nephrologists, pathologists, immunologists, pharmacologists, physiologists and cell biologists involved in renal research. It also provides a state-of-the-art review of the various pathogenic mechanisms underlying the progressive loss of renal function for all clinicians involved in the care of patients with chronic renal disease.
